Most residents choose a Continuing Care Retirement Community because they are well versed on the benefits we offer. At Village On The Isle, we know, and our residents know, that over the course of years there may come a time when they will need to transition to Assisted Living and perhaps eventually to Skilled Nursing. We make it our priority that this transition happens smoothly and that there are no surprises.
We are constantly communicating from day one with our residents about their health and well-being. We are actually quite aggressive when it comes to involving the family of our residents as well. Family members are invited and encouraged to attend meetings regarding their loved ones.
From the very first day a new resident moves into Village On The Isle, we start a plan for that resident. We set goals for each new resident moving in so we are constantly tracking and keeping up with their health and over all well-being. If and when certain identifiers start to show, such as un-cleanliness, confusion or inability to perform daily chores, we step in and discuss the transition of moving into assisted living. Resistance usually only comes about if the resident is suffering from other cognitive deficits which indicates a strong sign that they do need to transition.
